What is Support Coordination?

Support coordination is a service for NDIS participants that enables them to put their NDIS plan into action and make the most out of their NDIS supports and services.

Support coordinators assist you to understand and interpret your NDIS plan. The support coordinator can assist you in finding providers in the community.

QLSS Support Coordination services:

Level 1: Support Connection

Helping you get started and build the skills to connect with supports independently.

Level 2: Coordination of support

Ongoing coordination to help you manage multiple services and work towards your goals.

Level 3: Specialist Support Coordination

More intensive support for participants with complex needs or situations.

How can a Support Coordinator help me?

QLSS support coordinators can assist you to understand your priorities and support your NDIS goals through the following actions:

  • Support you I understand how to use your plan to reach your goals
  • Assist you to identify support providers in the community
  • Support to manage NDIS providers, including organising service agreements
  • Empower you to access the range of supports identified in your NDIS plan

  • Research and support linkage into other government services
